Council (ASC) press release on the SQ006 Accident Report

On October 31, 2000, at 1517 Coordinated Universal
Time (UTC), 2317 Taipei local time, Singapore Airlines (SIA)
Flight SQ006, a Boeing 747-400 aircraft, bearing Singapore
registration No. 9V-SPK,crashed on a partially closed runway
at Chiang Kai-Sheik (CKS) International Airport during takeoff.
At the time of the accident. There were 83 fatalities, 39
seriously injured, and 32 minor injuries.
The Aviation Safety Council (ASC) today releases the accident
report at 2PM at Civil Service Development Institute. (14th
Floor VIP room, section 3, Shin Sheen South Road, Taipei,
Taiwan, R.O.C.) Both Chairman Dr. Vicky Liu & Managing
Director Dr. Kay Yang , who is also the IIC of this investigation,
will hold the press conference.
After a year and half of investigation process included on
site, factual collection and analysis, the Aviation Safety
Council derives the conclusion and safety recommendations
as stated in the attached Executive Summary.
Therefore, based upon the analysis by the Safety Council,
there are 8 key findings related to probable causes, 36 findings
related to risk and 28 related to other findings. A total
of 60 recommendations will be issued to SIA, CAAS, Singapore
Government, CAA and MOTC (ROC), along with other recommendations
to the Boeing Company, International Civil Aviation Organization,
International Air Transport Association, FAA & JAA.
The Aviation Safety Council is an independent government
agency, responsible for civil aircraft accident investigation.
Its sole objective is to improve R.O.Cs aviation
safety. This objective is in full compliance with the 1994
Chicago Convention to the International Civil Aviation Organization
(ICAO) Annex 13. ASC is a continuous learning Institute with
the spirit of continuous improvement. Through the release
of this report, we sincerely hope from the lessons learned
of this tragic accident and by the implementation of the safety
recommendations will enhance aviation safety.
