Boss of transport firm fined S$70,000 for workplace accident

SINGAPORE: The boss of a transport supply company has been fined S$70,000 for a workplace accident, which resulted in the death of an employee.

Ministry of Manpower (MOM) prosecuted Steven Yen Tuck Hei, the sole proprietor of Yen Logistics Services, under the Workplace Safety and Health Act.

He was found to have failed to conduct any risk assessment for the work of loading and unloading cargo from trailers. The proprietor also did not provide documents on briefing drivers about safe work practices.

The accident occurred on 1 August 2007, at about noon, when Singaporean Koh Hong Son was transporting steel pipes to Pasir Panjang Wharves.

Koh released the securing strap so the pipes could be unloaded. But they rolled off, pinning Koh underneath. He was sent to hospital but subsequently died from multiple injuries.

MOM's investigations revealed that the bundles of steel pipes were stacked too high on the back of the trailer, creating a risk of the pipes rolling off during transportation and unloading.
