Seniors who need multiple long-term care services will soon benefit from a more seamless and better-coordinated experience. From April, they will need only one comprehensive assessment done by Integrated Community Care Providers (ICCPs) which were previously introduced in 84 sub-regions. A single community care plan will be developed for all providers to refer to. This will be rolled out from October. Also starting next month, more than 5,600 eligible seniors and their caregivers can benefit from the enhanced Home Personal Care (HPC+) service, with features such as home-based respite care and 24/7 tech-enabled monitoring. Senior Minister of State for Health Tan Kiat How said this in parliament on Thursday (Mar 5). He also announced that the Ministry of Health is setting aside up to S$45 million to help healthcare providers bear the costs of contributing to the National Electronic Health Record (NEHR). The NEHR Connect Grant will open for applications in July.
